Welcome to Washington University Peripheral Nerve Surgery website ("Site"). The University provides this site subject to the following terms of use and privacy policy (these "Terms"). If you use the Site, you accept these Terms. If you do not agree to all of these Terms of Use, do not use this Site. The University reserves the right, at any time, to modify these Terms. Purpose of this Site: The information contained in this Site is for educational and informational purposes only and should in no way be taken to be the practice of medicine or the provision of medical, nursing or professional health care advice, consultation, or services. The information should not be considered complete and should not be used in place of a visit, call, consultation or advice of your physician or other health care provider. Also, the information should not be used by health care providers as a substitute for their own judgments and decision making. The information obtained from this Site and any linked resources is not exhaustive and does not cover all diseases, ailments, physical conditions or their treatments.

Consultation on Patient Specific Diseases or Conditions: For your protection, we cannot answer any personal information via this web site about your medical condition or the medical conditions of your patients. However, our response to your telephone calls or e-mails concerning our general services or resources should not be interpreted as a recommendation for treatment or a professional consultation. Private Policy

Privacy of Information: We respect all the privacy of visitors to this site. Unless required for law enforcement or by statute, we will not share any information you supply solely to the University via this Site to any other outside unaffiliated entity without your consent. However, we may provide information to third parties that does not allow you to be identified or contacted. For example, we might inform third parties regarding the number of users of the Site and the most frequented resource pages.

Notwithstanding the foregoing, we reserve the right to work with third parties who provide services that may include, but are not necessarily limited to, website hosting, data analysis, hosted e-mail, and other administrative and technical services that assist us in maintaining the Site. In the event that we elect to work with third parties for these purposes, we reserve the right to share information you supply to us with such third parties, but only to the extent necessary for the provision of such services.

Washington University is not responsible for your release of information (including protected health information belonging to you or your patients) which you elect to share on any portions of the Site accessible by parties other than Washington University (e.g., chat rooms, forums, message board, etc.)

Visitor Tracking: We want each Site visitor to find useful information. In working toward that goal, we count our visitors and keep track of which pages our visitors select throughout the Site. To do this, we must use software called a "cookie." The "cookie" maintains numbers in aggregate form, and does not obtain personal information about any of our visitors.

This information helps us know what sections of our Site are visited frequently, so we can enhance those sections and ensure they are timely.

Indemnity: You agree to defend, indemnify, and hold Washington University, its respective officers, directors, employees, agents, licensors, and suppliers, harmless from and against any claims, actions or demands, liabilities and settlements including without limitation, reasonable legal and accounting fees, resulting from, or alleged to result from, your violation of these Terms of Use.

General Terms: The laws of the United States and the State of Missouri govern these Terms, without reference to conflicts of law principles. You agree that any legal action arising out of your use of the Site shall be brought exclusively in the state or federal courts located in St. Louis, Missouri, USA, and you hereby consent and submit to the exclusive jurisdiction and venue of these courts. If any provision of these Terms is determined to be unenforceable for any reason, that provision will be severed and will not affect the validity or enforceability of the remaining provisions. The failure of the University to enforce any rights under these Terms or to take any action against you in the event of any breach of these Terms shall not be deemed a waiver by the University as to subsequent enforcement of its rights in the event of a future breach.

Termination: These Terms shall remain in force until modified or terminated by the University. The University may suspend or terminate your access to all or part of the Site, at any time, without notice, with or without cause. The following provisions shall survive termination: Ownership, Acknowledgments, Restrictions, Disclaimer, Limitation of Liability, Freedom to Innovate, Submissions, Trademarks, Export, Indemnity, General and Termination.
